向学校写的英文请假信

我需要向学校请假，然后去问了老师，老师说得家长写一封信，但是从来没有学过，格式什么的一窍不通，求大家帮帮忙啊，万分感谢！
原因如下：
我是amy的父亲，在此我仅为她今年第四个学期假期（2013年10月28日至2013年11月27日）向学校请假至12月7日
1.为她修补牙齿（大概需要较长时间）
2.因她到澳大利亚学习到第一次回家时隔有18个月，希望接这次机会可以回中国过年，也希望她能同疼爱她身患癌症的老姑姑过最后一个新年。希望可以批准。
大家可以帮帮忙么，格式啊，语气尽量婉转一些，可以帮我稍微写的稍微长一点么，是写给校长的，谢谢大家了！。。。请给我一个完整的。。。

I'm Amy's father, I only for her this year fourth semester holiday (October 28, 2013 to November 27, 2013) to leave school until December 7th 1 to repair teeth for her (probably need a longer time) 2 for her to study in Australia to first home after a lapse of 18 months, that took this opportunity to return to China have the Spring Festival, also hope she can love her cancer-stricken old aunt last year. Hope can be approved.
